According to the California Highway Patrol, the crash happened around 6 a.m. on February 6 on the 110 Freeway in Los Angeles.
The accident involved Hernandez’s motorcycle, a BMW car, and a box truck. When first responders arrived at the scene, Hernandez was found lying on the road and was pronounced dead on the spot.
Reports say that while the BMW remained at the scene, the box truck driver allegedly fled before police arrived.
Family Asks for Public Help
Now, Hernandez’s family is asking the public to help identify and locate the driver of the box truck.
In an interview with local news station KTLA, Hernandez’s wife and children spoke about their loss and the pain they are going through.
His daughter, Marissa Hernandez, shared that her father had not ridden his motorcycle for some time and was excited to start riding again. On the morning of the crash, he left home early to ride to work.
A Mother’s Worry Turned Into Tragedy
Marissa said that her grandmother — Perry Hernandez’s mother — became worried when she did not receive her usual morning phone call from her son on February 6. That missed call was the first sign that something was wrong.
After several calls to him went unanswered, the family turned on the television and saw traffic backed up on the 110 Freeway. When news cameras showed the crash scene, they recognised his motorcycle lying in the middle of the road.
Later, authorities confirmed his identity through the medical examiner’s office.

Hernandez’s son, Perry Hernandez Jr., said it makes no sense that the truck driver allegedly left the scene while his father was lying on the road.
